Ecuador Diplomatic and Official visa checklist.
- No application form or photos are required.
- Submit a letter from sending department or agency, addressed to "Embassy of Ecuador, Washington D.C. ", explaining the purpose of the trip, dates of departure from USA, arrival in Ecuador, and departure from Ecuador, name of a contact person in Ecuador, name, passport number, and occupation of the traveler. Letter must be signed by the person that is in charge of that office or department.
- Send us a filled out checklist, passport and a letter from the sending department or agency.
